Dr. Ronald Savage has worked with children,
adolescents and young adults with neuro-developmental disabilities (i.e.,
acquired brain injuries, autism, developmental disabilities and other
neurological disabilities) for over 30 years. He is currently in private
practice and consulting with organizations providing services to children and
young adults with complex neuro-developmental disorders. He is also President
of the North American Brain Injury Society (NABIS) and Editor-in-Chief of Brain
Injury Professional. He is the Director of the TBI and Concussion Clinic at
the Center for Neurological and Neuro-Developmental Health (NJ). Dr. Savage
recently served as Executive Vice President for the Bancroft Neurosciences
Institute and directed the pediatric services division for Bancroft NeuroHealth
in New Jersey. He is the former Senior Vice President of Behavioral Health and
Rehabilitative Services at The May Institute in Massachusetts and the former
Director of Clinical Services for Rehabilitation Services of New York. In
addition, Dr. Savage has started, directed and consulted on several specialized
brain injury programs (inpatient and post-acute) for children and young adults
throughout the country.
Dr. Savage has taught at the elementary and
secondary school level as a special educator, as well as positions at several
colleges and universities including Professor of Education (tenured) at
Castleton State College, Clinical Professor of Neurology at the University of
Medicine and Dentistry of New Jersey, and Associate Professor of Psychology at
Rutgers University. He also held adjunct academic appointments at Temple
University; Tufts University, Department of Rehabilitation Medicine;
Northeastern University, Department of Psychology; and Program Consultant to The
George Washington University, Master’s in Special Education and Brain injury
program. Dr. Savage serves on the Editorial Board of the Journal of Child
Neurology. He was a Founding Member and Chief Examiner of the American
Academy for the Certification of Brain Injury Specialists.
Dr. Savage is the former Chairperson of the
Pediatric Task Force for the Brain Injury Association of America; the past
Co-Chairperson of the International Pediatric Task Force for the International
Brain Injury Association; and serves on several professional advisory boards
across the country. In 1989-90, Dr. Savage spearheaded the campaign to include
traumatic brain injury (TBI) in Public Law 101–476: Individuals with
Disabilities Education Act. He also worked with the Emergency Medical Services
for Children (EMSC) training programs; the Office of Special Education and
Rehabilitation Services of the United States Department of Education; the
Professional Advisory Task Force for the National Council on Disability for the
United States Congress; and the advisory board for the Traumatic Brain Injury
Act. In addition, he is helping state education departments write guidelines
for students with neurological injuries and disabilities.
Dr. Savage has presented at over 200
conferences, training seminars, and hospital grand rounds presentations in the
past 4 years. He has published numerous articles, chapters, manuals and books
on children, adolescents and adults with acquired brain injuries and
neurological disabilities. His professional memberships include: Board of
Directors, Neurotrauma Registry; Member, American Society of
Neurorehabilitation; Member, American Psychological Association; Member,
National Association of School Psychologists; Member, National Rehabilitation
Association; Member, Case Management Society of America; Member, North American

Title: Impact of TBI on the continued neurological development of
children, adolescents and young adults.
Description:
This lecture presents data and research on the long term effects of
traumatic brain injury (TBI) on children as they continue to develop and grow.
Neuro-maturational stages of development in children, as identified through
medical imaging technology (CAT, MRI, PET) and neuropsychology, will be
presented. Research demonstrates that very young children, infants, toddlers
and preschoolers, are particularly vulnerable to TBI because their brains are
in the early, rapid stages of brain development and they have limited cognitive
reserve. Recovery from severe brain injury for many children may be limited to
the skills that were already established at the time of their brain injuries.
In addition, recovery of previously acquired skills may not necessarily insure
continued development of new and later emerging skills or skills in a rapid
state of development at the time of injury (Savage, 1999; Chapman et al., 2004;
Ewing-Cobbs, Barnes, & Fletcher, 2003, Anderson, 2007, Savage 2009).
Furthermore, the frontal-temporal regions of the brain have
accelerated development in the later adolescent and young adult years.
Unfortunately, many children, adolescents and young adults sustain injuries to
their frontal-temporal brain regions and, as a result, experience long term
deficits in executive functioning skills, personality changes, and behavior
deficits. These deficits will further negatively impact functioning in their
adult years. Lastly, this lecture will present program models and therapeutic
strategies to support children, adolescents and young adults through these
developmental stages both in hospital and community.

Context: The
relationship between cerebellar tonsillar ectopia (CTE) and head and neck
trauma is poorly understood.
Objective: To assess the frequency and degree of CTE in traumatic versus
non-traumatic populations via recumbent and upright MR imaging.
Design: Case-control. Two radiologist readers blinded with regard to the study
group status of each scan.
Setting: Four outpatient radiology centers.
Patients: Cervical MRI scans for 1200 consecutive neck pain patients ≥15 years
of age were obtained and reviewed; 600 trauma (cases) and 600 nontrauma
(controls). Half of the cases and controls were scanned in a recumbent position
and half were scanned in an upright position.
Main Outcome Measure: Level of cerebellar tonsils relative to the plane of the foramen magnum.
Results: A total of 1195 of 1200 scans were considered interpretable for
cerebellar tonsil level, grouped in “stations.” The most cephalad mean tonsil
station was found in the two non-trauma groups (mean station of 1.1 and 0.9,
respectively). The recumbent trauma group was next lowest at 0.7, and the
lowest station by far was the upright trauma group at a mean station of 0.1 (p
= <0.0001). Significant Intergroup differences were seen in the proportion
of scans with CTE; 5.7% and 5.3% in the recumbent and upright non-trauma
groups, respectively, versus 9.5% and 23.7% in the recumbent and upright trauma
groups (p = 0.0001).
ConclusionThe results described in the present investigation are first to
demonstrate a neuroanatomic difference between neck pain patients with and
without a recent history of trauma. Upright MR imaging appears to increase the
sensitivity to CTE in recent trauma cases by a factor of 2.5. Further
investigation including biomechanical study of the effects of whiplash trauma
on the ligamentous support of the upper cervical spinal cord and brain as well
as clinical studies correlating CTE with symptom severity, distribution, and
persistence are needed.
